To begin the enrollment process, please fill out the "Enroll Now" form to submit an online enrollment application. Upon receipt of this form, we will contact you regarding the next steps to set up a tour and enroll your child if there is space. We usually respond to these requests via e-mail, so please enter an e-mail address you check regularly. If you are enrolling two or more children, simple fill out information for one of them on the form to the right, and when we contact you, let us know you are looking to enroll more than one child.
If you would like to save yourself a step and download our full enrollment packet to bring in when you visit our building, please click the link below the enrollment form. Discovery Academy cannot hold a place until all the necessary paperwork has been turned in to the school office.
The school accepts applications for enrollment throughout the school year, during which any resident of Lucas and its contiguous counties may apply. Students are enrolled on a first-come, first-served basis. The academy does not charge tuition nor discriminates in admissions policies or practices on the basis of intellectual or athletic ability, measures of achievement or aptitude, or status.
Documents you will need to bring with you when you come to enroll:
Student Birth Certificate
Student Social Security Card
Student immunization records (can be faxed by doctor's office)
Two proofs of residency
Guardianship paperwork (if being enrolled by non-birth parent)
IEP if applicable
ATTENTION: Our enrollment policy states that student must be five years of age by no later than September 30th to enroll in kindergarten or six years of age no later than September 30th to enroll in first grade.
